The objective of this review is to analyze the published literature about the importance of interleukin IL- 1β polymorphism as a genetic marker of periodontal illness. The evidence shows variability in natural history and therapeutic response of this pathology within patients. Genetic and immunoinflamatory factors could determine diagnosis, prognosis and treatment strategy of the compromised patients. IL-1 acts as the key stone in the regulation of immunoinflamatory response and its polymorphism has been studied in many populations. A literature search in PubMed and Cochrane from 1995 to 2006 was performed. In some studies of humans with severe periodontal destruction and IL- 1high levels, a polymorphism of the gene that codes this cytokine was detected. The most frequent polymorphisms are IL-1β-889 and IL-1β+3954. The sole presence of these polymorphisms does not cause the illness, but can accelerate and worsen the process, increasing the inflammatory response when there is a dentobacterial plaque. Based on the present knowledge, there is a lack of evidence that supports IL-1β polymorphism as a risk factor to develop periodontal illness.
