    • ...The care of patients with multiple brain metastases is a fast-growing indication for radiosurgery. Two main problems associated with radiosurgery for these patients have been the relatively long treatment times required to treat each patient and the difficulty of irradiating tumors located at the extreme limits of the brain. In earlier models the stereotactic coordinates were set either manually using trunnions or robotically with an automatic positioning system (APS). APS technology obviated the need to manually adjust each set of coordinates for a multiple-isocenter plan. However, APS had a narrower range in the x (59–141) and y (40–160) dimension. Because of the limitations of the x and y coordinate range of APS, tumors located outside this range needed treatment using the trunnion mode. Thus in a patient with multiple metastases usually some tumors could be treated with APS but others required manual setup for x, y and z coordinates (trunnion mode). The change over form APS to trunnion mode and manual coordinate setup on an average took 10 and 6 min, respectively. This led to longer treatment times for these sick patients. Certain tumors that fell outside the range of the trunnion mode in the x (50–150), y (25–175) or z (40–166) coordinate could not be treated.
