Abstract  A classification system for a specific disease should be as simple as possible, comprehensive and reproducible in order to understand the natural history and to formulate a correct diagnosis and suggest appropriate therapy. As for rotator cuff diseases this was not always possible for the great anatomical complexity and also for the lack of adequacy of imaging in the past. Arthroscopic technique has improved the correct identification and classification of rotator cuff lesions and associated lesions.
