Summary The critical functional importance of the thumb makes volar plate or collateral ligament injuries extremely disabling. Based on the evaluation of 38 cases with X-ray documented instability it was shown that pre-arthritic deformity can be prevented by immediate repair (31 cases) using a V-hook wire suture or replacement with a tendon graft (4 cases) or with lyophilic meninges (2 cases) in late repair. In one case the ruptured volar plate was reinforced by tendon transposition. All patients showed good results in power grip and pinch, but a little less flexibility.
